Attention all USRowing 2007 Pan American Game Hopefuls!

The United States Olympic Committee has requested that USRowing submit a long and detailed list of potential athletes and staff for the 2007 Pan American Games.

ALL POTENTIAL MEMBERS OF THE USA DELEGATION (athletes, coaches, team leaders, officials, team administrators, etc.) MUST COMPLETE AND SUBMIT THE FOLLOWING TO USRowing NO LATER THAN FEBRUARY 9, 2007. Potential athletes and staff who do not complete the necessary paperwork face the possibility of not being allowed to participate in the 2007 Pan American Games.

1) USOC REGISTRATION FORM, which MUST be completed online here: https://events.usoc.org/PanAm07Registration.

2) USOC GAMES FORMS (Code of Conduct, General Release, Grievance Procedures and Background Search Requirement) -- signature required.

NOTE: On Attachment A of Code of Conduct (pg. 17), please sign your name and write “N/A” if that is the case.

3)  USOC MEDICAL HISTORY QUESTIONNAIRE (athletes and staff) -- signature required.

4)  HIPAA (Health Insurance Portability and Accountability Act) FORM -- signature required.

5)  ATHLETE’S BIOGRAPHICAL QUESTIONNAIRE.

6)  LEGIBLE COPY OF PICTURE PAGE OF PASSPORT (IMPORTANT: Passport must be valid for at least six (6) months from end of competition (1/19/08).

7)  ONE DIGITAL HEADSHOT SUBMITTED ELECTRONICALLY IN JPEG FORMAT (30-100 KB) -- Note: The background of this photo MUST be light in color (white or light gray). Hats, headbands and dark glasses should not be worn in the photo. Please, no funny facial expressions!
